** As per Practice Note 5 of 2009, if the proposed name have an association with some entity already registered and active on the CIPRO database, the following documents need to be presented:

Written proof of an original letter signed by the most senior officer(s) of the cc or company on the cc/company’s letterhead

A certified copy of the identity document of the director(s) or member(s).

Form CK1 canbe completed in capital letters, be typewritten, lithographed or
printed in legible characters with black ink.

Where a person signs on behalf of a member, power of attorney must be attached.

Minors (persons under 18 years of age), children, and other persons
under legal disability must be assisted by their parents, guardians or
representatives as the case may be, and the capacity must be stated.

If no identity document has been issued, a written statement to this effect
must be attached. In case of foreigners, a passport and a copy thereof, and a
written letter stating he/she is not a South African citizen will be needed.

Surname. (If juristic person, mention name and capacity. If trustee, also
mention name and particulars of testamentary trust. If acting nomine officii as
trustee, administrator, executor, curator, state capacity.)
